This study was done to assess the prevalence of cancer of the prostate and the value of percentage free/total Prostate Specific Antigen (PSA) in the intermediate PSA range. One hundred and five patients aged greater than fifty years, seen over a period of one year with total PSA values between 4-10 ng/ml and normal digital rectal examination findings of the prostate were studied. Assay for total and free PSA was done using an enzyme linked immunosorbent assay (ELISA) kit (TECO INC,USA).The percentage free/total PSA (%F/TPSA) was calculated and all subsequently had digitally guided trans-rectal six core biopsy of the prostate. The results of the study showed that the prevalence of cancer of the prostate among patients with palpable benign prostate gland, in the intermediate PSA range was 13.3%. The sensitivity was highest (100%) at 40% with minimal loss in specificity (93.4%) and positive predictive value (70%). I recommended that 40% F/TPSA cut off should be used for prostate biopsy among patients with intermediate PSA values and normal digital rectal examination findings of the prostate. The prevalence of cancer of the prostate and the value %F/TPSA among patients with intermediate PSA values and normal digital rectal examination findings of the prostate should be made aware to medical practitioners. This is to aid decision taking with regards to referral and subsequent prostate biopsy for patients in this category. A larger study incorporating trans-rectal ultrasound guided prostate biopsy is recommended to collaborate these findings.
