HAS SUBCHORIONIC HAEMATOMA ANY PROGNOSTIC SIGNIFICANCE IN NIGERIAN PREGNANT WOMEN?

Abstract

INTRODUCTION Pregnancy loss at any gestation is associated with severe emotional morbidity. Threatened abortion is associated with a lot of anxiety and apprehension to preserve the pregnancy both by the women and the attending obstetrician. The aetiology of pregnancy loss varies. Patients with intrauterine haematoma are said to belong to high-risk group for the remainder of their pregnancies. 1 In addition to signs of fetal life on sonography, sub-chorionic bleeding is an important factor affecting the outcome of gestation in patients with clinical threatened abortion2 . In Africa, where high social premium is placed on childbearing. I investigated the prognostic significance of sub-chorionic haematoma found in threatened abortion on pregnancy outcome. This, I believe, will assist the practicing obstetrician and sonologist in offering a guided prognostic advice on pregnancies complicated by sub-chorionic haematoma. PATIENTS AND METHOD Five hundred and thirty six (536) patients with early first trimester bleeding in singleton pregnancies had abdomino-pelvic ultrasound examination on 4 them by a consultant radiologist/sonologist. Forty-seven of them were found to have sub-chorionic haematoma. This was diagnosed when there is an extra- chorionic crescent anechoic or complex collection. The fetal heart sounds were still present, in all these cases. Those with ultrasonographic evidence of sub-chorionic haematoma were subsequently followed up for the rest of the pregnancy.
