ACCURACY OF SURGEON PERFORMED BREAST ULTRASONOGRAPHY IN THE ASSESSMENT OF BREAST LUMPS IN FEMALES

Abstract

Breast lump is one of the common modes of presentation of breast disorders in females. It causes great anxiety because of the fear of cancer. Triple assessment (clinical-imaging-pathological) which is the recommended approach to breast lump is not commonly practice by Surgeons in our environment. This study is aimed at determining the diagnostic accuracy of Surgeons in performing own breast ultrasonography while assessing breast lumps at a Nigerian Teaching Hospital. Breast ultrasonography of both breasts were done for 73 women by the Surgeon and Radiologist independently. These lumps were subjected to histology or cytology appropriately. The pathological diagnoses were compared with Surgeon’s and Radiologist’s ultrasonographic diagnoses to determine their diagnostic accuracy. The sensitivity, specificity and diagnostic accuracy of the Surgeon were 60%, 100% and 96.9% respectively for malignant breast lumps. With a confidence interval of 35.9-97.9.For benign breast lumps, the Surgeon had a sensitivity of 98%, specificity of 86.9% and diagnostic accuracy of 94.5%. The Radiologist had similar figures for benign lesion but a sensitivity of 80%,specificity of 100% and diagnostic accuracy of 98.5% for malignant breast lesions.With a confidence interval of 41.7-98.9 These results show that Surgeon performed breast ultrasonography is associated with high diagnostic accuracy. It is therefore recommended that Surgeonperformed breast ultrasonography be considered an initial investigation in the evaluation of breast lumps
